Monthly Plan
Billed every 30 days. Each cycle includes one month’s supply of medication.
Quarterly Plan
Billed every 90 days. Each cycle includes a 3-month supply. A new intake form is required before each quarterly renewal for GLP-1 (Tirzepatide) and after 6 months for all other products.
First Charge
You are charged at checkout when you complete the initial intake form and place your order.
Renewal Charge
Subsequent charges occur automatically at the start of each new billing cycle (30 or 90 days from prior charge).

Initial Intake (All Products)
You must complete an intake form at sign-up before your first order is processed. This is required to generate a prescription or treatment plan.
GLP-1 Recurring Intake
Your GLP-1 prescription (Tirzepatide) expires every 90 days. When your prescription is approaching expiration, you will automatically receive a follow-up intake form to complete. Your provider will review your responses and re-issue your prescription before your next shipment is released.
Monthly patients: billed every 30 days, but the intake form is triggered at the 90-day prescription expiry (approximately every 3rd billing cycle).
Quarterly patients: billed every 90 days, so the intake form aligns with each renewal.
If you do not complete the intake form, your subscription may be paused or cancelled until it is submitted.
All Other Products
Your prescription expires approximately every 6 months. When it’s time to renew, you will automatically receive a follow-up intake form. Once completed and approved by your provider, your subscription will continue uninterrupted.

Option 1 — Patient Portal (Self-Serve)
Log into your account at www.upgradeyourhealth.net and click on “Portal Login” from the home page. You will use the same user name and password you set up when the product was originally ordered.
Navigate to “Subscriptions” or “My Plan.”
Select “Cancel Subscription” and confirm.
A cancellation confirmation email is sent immediately.
Option 2 — Contact Upgrade Your Health
Contact us at info@upgradeyourhealth.net and we can request the cancellation for you. You will receive a confirmation email.
Important: To avoid being charged for the next cycle, please cancel before your next scheduled billing date. Cancellations after a charge has processed will not be refunded.

Intake Form Not Completed
For a GLP-1 (Tirzepatide) if you do not complete your required intake form within the grace period following the reminder notifications, the system will flag the subscription and it may be auto-cancelled or held until the form is completed.
Failed Payment
If a renewal charge fails (e.g. expired card, insufficient funds), the system will attempt to retry. After a defined number of failed attempts, the subscription is cancelled and you will be notified.
Provider Denial
If the prescribing provider reviews the intake form and does not approve the next cycle, the subscription will not renew.
Patient Request via Portal
Immediate cancellation per patient action. See “How to cancel your subscription?”

You have access to a self-service portal to manage the following:
View current subscription status and next billing date
Update payment method (credit card, billing address)
Complete or re-submit intake forms
View order history and tracking
Download or view receipts
Message your care team
Cancel or pause their subscription
You cannot change your treatment plan, request a dosage change, or access prescription records directly through the portal.

mg/mL is the concentration. For example a tirzepatide + B12 concentration of 10mg/0.5mg/mL means there is a concentration of 10mg of tirzepatide and 0.5mg of B12 for every mL in a vial.
All tirzepatide vials are 2mL total volume and all weekly dosages are for 50 unit injections. This means, for the example above, the weekly 50 unit dosage will be 5mg of tirzepatide and 0.25mg of B12.
